Revisión Narrativa Hepatitis A: Epidemiología y transmisibilidad

Resumen (ES)

La hepatitis A es una enfermedad de curso generalmente agudo que afecta principalmente al hígado, causada por el virus de la hepatitis A (VHA), un picornavirus envuelto y estable, capaz de resistir condiciones adversas. La transmisión ocurre por contacto interpersonal por vía fecal-oral, mediante el consumo de agua y alimentos contaminados. Se realizó una revisión con el objetivo de describir la capacidad transmisora del virus y la problemática que representa. Para esta revisión bibliográfica fueron seleccionados artículos científicos originales, casos clínicos y boletines epidemiológicos relacionados a la infección por VHA a escala mundial y nacional. El VHA es el causante más común de hepatitis viral aguda a nivel mundial, la infección producida ocasiona un proceso necroinflamatorio agudo que se resuelve espontáneamente sin secuelas crónicas con un cuadro clínico de leve a severo y un periodo de incubación variable con una resolución completa de la enfermedad en >99% de los casos y bajo porcentaje de recaídas. Anualmente se estima que 1,5 millones de personas son infectadas por VHA, en el 2016 en Venezuela se reportaron 4305 casos sospechosos.  La incidencia poblacional de la infección está relacionada con factores socioeconómicos como saneamiento y calidad del agua, por tanto, mejoras en estas condiciones conllevan a un cambio en la susceptibilidad al virus, desde edades tempranas hasta adultos mayores. La tasa de infección del VHA disminuye al mejorar las condiciones sanitarias básicas de una población, siendo posible su prevención y control al implementar un plan de vacunación contra el virus de la hepatitis A.

Resumen (EN):

Hepatitis A is a disease with a generally acute course that mainly affects the liver, caused by the hepatitis A virus (HAV), a stable and enveloped picornavirus capable of resisting adverse conditions. Transmission occurs through interpersonal contact via the fecal-oral route, through the consumption of contaminated food and water. A review was carried out with the objective of describing the transmitting capacity of the virus and the problems it represents. Original scientific articles, clinical cases and epidemiological bulletins related to HAV infection were selected for this bibliographic review on a global and national scale. HAV is the most common cause of acute viral hepatitis worldwide, the infection produced causes an acute necroinflammatory process that resolves spontaneously without chronic sequelae, with a mild to severe clinical picture and a variable incubation period with complete resolution of the disease in> 99% of cases and low percentage of relapses. Annually it is estimated that 1.5 million people are infected with HAV, in 2016 in Venezuela 4,305 suspected cases were reported. The population incidence of the infection is related to socioeconomic factors such as sanitation and water quality, therefore, improvements in these conditions lead to  a change in the susceptibility to the virus, from early ages to older adults. The HAV infection rate decreases by improving the basic sanitary conditions of a population, making it possible to prevent and control it by implementing a vaccination plan against the hepatitis A virus.
